What the minerals actually do inside your machines
Hard water carries dissolved calcium and magnesium. Heat that water or let it sit still, and the minerals drop out and stick to metal. In a water heater they settle on the bottom as a crusty layer. In a dishwasher they coat the heating element, the spray arm holes, and the fill valve screen. In a washer they build up in the hoses and inlet screens. Think of scale as a blanket. It traps heat where you don't want it and blocks water where you need it. Parts run hotter, cycles run longer, and pumps push against restriction. That extra work is what shortens the life of the appliance, not one dramatic failure.
The habit that prevents most of it
Run a hot descale cycle on anything that heats water. For a dishwasher, put a cup of white vinegar or a citric-acid dishwasher cleaner in the bottom of an empty machine and run the hottest normal cycle. For a washer, use the tub-clean setting with a machine cleaner, and don't mix bleach and vinegar. While you're in there, wipe the door gasket and pull the dishwasher filter basket, rinse it under the tap, twist it back in. Use a rinse aid too. It isn't a gimmick. It helps water sheet off dishes instead of drying into spots and film.
How often, based on your water
With moderately hard water, descale the dishwasher every three months and run the washer tub-clean monthly. In harder areas, and that covers much of the Sacramento Valley, the foothills, and plenty of well-water homes, move the dishwasher to every six to eight weeks. Rinse the dishwasher filter every two weeks if you don't pre-rinse dishes. Water heaters want a flush about once a year. If yours is tankless, follow the descaling interval in the manual, since scale hits those faster. A softener helps, but check the salt monthly. An empty brine tank means you've been on hard water for weeks without knowing it.
Signs prevention time has passed
Glasses that stay cloudy after a vinegar soak are etched or heavily scaled, not dirty. Cycles that stretch far past normal usually mean the heating element is insulated by mineral and can't get to temperature. Spray arms with clogged holes leave whole sections of the rack dirty. A water heater that rumbles, pops, or runs out of hot water early generally has a thick sediment bed. Washers that fill at a trickle often have mineral in the inlet screens, and ice makers making small hollow cubes are fighting scale in the fill tube. At that point a part needs cleaning or replacing, and another maintenance cycle won't do it.

Is vinegar safe for my dishwasher?
An occasional vinegar descale cycle in an empty machine is fine for most dishwashers. Don't do it every week, and don't mix it with bleach. If your manual says no acids, use a citric-acid dishwasher cleaner instead.
Will a water softener fix damage that's already there?
No. A softener stops new scale from forming, but the existing buildup stays put. You'll still need to descale once or twice after installing one, then keep the brine tank filled and check it monthly.
How do I know if my water is hard?
Look for white crust on faucet aerators and shower heads, soap that won't lather, and film on glassware. Your water utility publishes a hardness number, or a cheap test strip answers it in seconds.
